
Short answer: Most tankless water heater problems come down to one of five things: scale buildup on the heat exchanger, ignition or flame failure from a gas or venting issue, a flow rate too low to trigger the unit, a failed flow sensor, or a blocked intake or exhaust vent. Scale is the most common by a wide margin, and in hard water areas it is the reason a unit that worked perfectly for three years starts running cold. Most of these are genuine repairs. A cracked or corroded heat exchanger on an older unit is usually the point where replacement makes more sense.
Tankless units are genuinely good. They also fail differently from tank heaters, and the failure modes confuse people because there is no tank to look at and no obvious puddle on the floor.
Here is what actually goes wrong.

Scale is the main event
If you take one thing from this, take this one.
A tankless heater works by running water across a heat exchanger and heating it hard and fast. Minerals in the water, mainly calcium and magnesium, deposit on that exchanger as scale. Scale insulates. The unit has to work harder to reach the same output temperature, efficiency drops, the exchanger runs hotter than it should, and eventually the unit starts throwing overheat errors or simply cannot keep up.
This is why a tankless heater that was perfect for a few years starts behaving badly. Nothing broke. It silted up.
This matters a lot in North Idaho. A large number of homes here are on well water, and well water in this region is frequently hard. Even on municipal supply, hardness varies considerably across the area. A unit on hard water needs descaling more often than the manual’s default assumption.
The fix is a flush, sometimes called descaling: circulating a descaling solution through the exchanger to dissolve the deposits. On hard water, annually is a reasonable rhythm. On very hard water, more often. Manufacturers typically require this maintenance to keep the warranty intact, which is worth knowing before a warranty claim.
If you are descaling frequently, the better long-term answer is treating the water. A water softener or filtration system protects the heater, and everything else plumbed downstream of it.
The winter problem nobody explains properly
This one gets misdiagnosed as a fault constantly.
A tankless heater does not store hot water. It raises the incoming water temperature by a certain number of degrees as it flows through, and its capacity is expressed as a temperature rise at a given flow rate.
In summer, groundwater coming into a North Idaho home might be somewhere in the fifties. In deep winter it can be down around 40 degrees or colder. If you want a 120-degree shower, the unit has to lift the water a lot further in January than it does in July.
Because the physics is fixed, the unit does the only thing it can: it reduces flow to maintain temperature. So your shower goes from strong to weak, or from hot to lukewarm, and only in winter.
That is often not a broken heater. It is a unit sized for a milder inlet temperature than North Idaho actually delivers. Which means the repair conversation is different: descaling may help at the margins, but if the unit was undersized for winter conditions in the first place, no repair fixes that. You either accept reduced winter flow, reduce simultaneous demand, or move to a larger unit.
Anyone who tells you a winter-only flow drop is definitely a fault has not asked the right question.
Ignition and flame failure
If the unit is throwing an ignition or flame error, the usual suspects are:
- Gas supply. Insufficient gas pressure or an undersized gas line. Common when a tankless unit replaced a tank heater without the gas line being upgraded, because tankless units demand far more gas while firing
- Venting. Blocked, restricted, or improperly terminated. Snow and ice blocking an exterior vent termination is a real North Idaho failure mode, and it is worth checking after heavy snow before calling anyone
- Igniter or flame sensor. Wears out, gets dirty, replaceable
- Condensate blockage on condensing units. The condensate line can clog or freeze
The snow one is worth repeating because it is free to check. If your vent terminates outside at low level and you have had drifting snow, go and look.
Flow rate and the minimum activation threshold
Tankless units need a minimum flow before they fire, often somewhere around half a gallon per minute. Below that, the unit will not activate and you get cold water.
This shows up as: a low-flow bathroom faucet that never runs hot, or hot water that cuts out when you throttle the shower back. It is not always the heater. A clogged aerator, a partly closed valve, a failing mixing cartridge, or a sediment-blocked inlet filter screen can all pull flow below the threshold.
The inlet filter screen is worth mentioning because it is easy to forget and it does clog, particularly on well water.
The cold water sandwich
Worth naming so you can stop worrying about it.
You run hot water, turn it off, then turn it back on shortly after. You get hot, then a short burst of cold, then hot again. That is water that was sitting in the pipe, followed by the gap while the unit fires back up.
It is normal tankless behavior. A recirculation system reduces it. It is not a fault and it does not need repairing.

Tankless units generally last longer than tank heaters, often around 20 years against 10 to 12, and that longevity is a real part of their value. But it depends heavily on whether they were maintained, which brings it back to descaling.

- Treat hard water if you have it, rather than fighting the symptom every year

Regular plumbing maintenance is unglamorous and it is the difference between a tankless unit lasting five years and lasting twenty.
